Hearing Dog Candidate Testing Request
By popular request, we have designed a Request for Testing Form for a dog you feel may be qualified to become a Companion Hearing Dog.
This form is specially designed to supply us with all the information necessary to make the process as smooth and sure as possible.
If you have questions about the form, please email them to TJ at tjatadogears@aol.com and be sure to put "Dog Ears" in the subject line so she doesn't delete your email. If you need to call instead, you may call 405-478-2303 at Anne's Country Club for Pets and ask for TJ.
Dog's Requirements
The dog must be at least 9 months of age, no older than 2 years of age, have had at least it's rabie's vaccination, be friendly to people and other dogs, heartworm free (recently tested), healthy, willing to work with you, and interested in appropriate sounds.
What You Will Need to Complete the Form
You will need copies of veterinary records. The rest is part of the form.
